To help make your visit as smooth and stress-free as possible, we encourage all new patients to complete their paperwork ahead of time. You can download and fill out our New Patient Forms at home, then bring them with you to your child’s first appointment. This helps streamline your check-in process and ensures we have all the information we need to provide the best possible care.
If you're unable to complete your forms in advance, no problem—just plan to arrive about 15 minutes early so we can assist you with any remaining paperwork and verify your insurance details.
Our forms include important information about your child’s medical history, dental history, and insurance coverage. Having this information ready allows our team to tailor your child’s care and make the most of your time in the office.
